If you are a Prime or Upgrade member and need to file a claim for a loss while on a covered trip to a Resort Club or Exchange, select a link below to download claim forms. To avoid a delay in processing your claim, be sure to include all requested documentation with your completed claim forms including, but not limited to, proof you were on an eligible Trip, receipts, medical documentation, explanation of benefits, and proof of loss.
